Welcome Back Letter

 Dear Families,
We are so excited to welcome back our returning families and meet many new families on September 4th!  Please review the information below in preparation for our opening day.
 
Early Drop Off:
Our fee-based Early Drop Off program begins September 4th.  The form to register can be found on our website at www.psis104.com on the Home Page.  No student can begin the program until the forms and payment are submitted.  All students participating in the Early Drop Off program meet in the Main Building at 9115 5th Avenue. 
 
Arrival/Dismissal: 
  • Kindergarten students will enter the building through the Annex Main entrance (415 89th Street) at 8:30 a.m. with their parents.  Parents are welcome to stay briefly to meet the teacher. A welcome event and brief orientation will take place in the cafeteria at 9 AM. 
  • Grade 2 students -will enter the building through the 5th Avenue entrance to the left of the Main door at 8:30 a.m. and meet their teachers in the auditorium.
 
All other students will enter at 8:10 AM.  The breakfast will be a Grab and Go served to any student as they enter each morning. 
Beginning September 5th- All students enter at regular time 8:10 AM  
Dismissals will begin at 2:25 PM.

Forms your child should bring on the First Day of School:
Attached please find the forms that were sent home in June.  These forms should be completed and sent in September 4th with your child/children:
(1) Emergency Contact Card:
            Please be sure to add all parent/guardians first and last names, working phone numbers for any adult that may pick up your child, signatures of parent/guardian- **Please do not check that it has been completed in NYCSA
(2) Media Consent Form
(3) Arts Permission Form
(4) Housing Questionnaire
(5) Bus forms- Only for those requesting Yellow Bus (K-6) or Metro Cards
 
Student/Parent Handbook:
Please review the Student/Parent Handbook with your child that is posted on our school website on our home page.  This handbook contains information about many school policies including:
  • Attendance
  • Cell Phone Policy (Updated)
  • Dress Code
  • Discipline Code
CELL PHONE POLICY UPDATED:
Attached please find the updates School Cell Phone Policy.  This policy is for all students K-8. 
 
Family Income Inquiry Forms:
We are asking all families to complete the Family Income Inquiry Form.  This can be done online at www.myschoolapps.com  All families must complete this form-it is directly tied to funding for our school!
NIA Afterschool:
The fee-based NIA program for grades K-5 begins September 4th.  Only students that have registered with NIA can begin.  The NIA program for grades 6-8 does not begin until September 15th.  Registration forms for grades 6-8 are on our school website at www.psis104.com in the “NIA” tab.  For questions or Grades K-5 registration, please call (718) 836-4630 X 122. 
 
GRP Letter:
Attached please find the General Response Protocol Parent Letter that outlines the emergency readiness initiatives.  We will be conducting our first Emergency Drills at the Main Building and the Annex the week of September 8th.  We will review Emergency Procedures with students prior to the drill.    Translations are available at:  https://infohub.nyced.org/nyc-doe-topics/students-and-families/safety-and-emergency-preparedness/general-response-protocol
